Foundations Health’s substance use treatment program fosters a culture of healing and community in which individuals feel empowered to seek a new healthy lifestyle.

Admission to the outpatient treatment program is based on presenting symptoms, willingness to follow program requirements and an apparent need for treatment.

After an assessment, each patient receives an individualized treatment plan that meets their specific needs. Every person is unique, and we work with you to assess and determine the best treatment options for you.

Break the Stigma
Addiction recovery is possible, although people may put off the decision to get help for a variety of reasons. If you or a loved one is in need of help, realize there is a light at the end of the tunnel and help is available if you are ready to take action.
